How to create ticket purchase questions (Survey Questions)

Event pre registration form

The pre registration form in Bevy enables directors with appropriate permissions to add questions, on a per event basis, to a form that displays when a member RSVPs or purchases a ticket to an event.

Note: Based on the data privacy and data protection laws, chapters are not allowed to collect email addresses, phone numbers in the event pre registration form.


Editing the pre-registration form

Questions are added to forms via the dashboard, and must be added to each event form - they don’t carry over from event to event.

  1. Select the event within the chapter dashboard
  2. Click the “ Form”tab
  3. Click “ Add new field”
  4. Type the question or appropriate text
  5. Change the question type using the gear icon
  6. Choose from the following question types
    1. Short Answer
    2. Paragraph
    3. Email
    4. Number
    5. URL
    6. Checkbox
    7. Radio
    8. Multiple Checkboxes
    9. Dropdown
    10. Multiple Choice
  7. Select
  8. The Copy icon will allow you to duplicate questions


    Note: Questions included by HQ can’t be removed by chapter directors.

You can add, remove + drag and drop the order of questions.

Accessing the Form Data

To Access the information - download the CSV file of that specific event, and all the fields will be in there. 

You can do this before, during and after the event
